The Outpatient Chemical Dependency program is an OASAS licensed Part 822 Program serving adults 18 and over with Substance Use Disorders and co-occurring disorders. The Social Worker is an integral member of the Team and provides clinical expertise in various treatment modalities that are evidenced- based and patient centered. Our services include evidenced based practices, Medications for Opioid Use Disorder and Medications for Alcohol Use Disorder, combined with group and individual counselings. The Social Worker provides sepcialized groups as well. Responsibilities include assessment and individualized treatment planning, individual and group treatment, education, referral and discharge planning with linkages to community agencies for patients, their families, and/or significant others. The Social Worker is available as a resource for assessing patients in various settings including Outpatient Care, and the Hospital setting to increase access to care and engagement in treatment. The Social Worker works closely with community agencies and collaborates to deliver quality and patient centered care for our patients. Also available to provide presentations and education to the Community and Hospital staff. Role may include supervising students. LCSW, current NY State license to practice social work. Knowledge of theories, therapies and techniques of evidenced based clinical work for adults with Substance use disorders (SUD) as well as co-occurring disorders. Familiarity with OASAS regulations and requirements. Experience working in a Clinical setting with patients with SUD. Strong working knowledge of community resources, and agencies to enable a holistic approach to care for patients and their families/significant others. Excellent communication and interpersonal skills demonstrable in interactions with staff, patients, visitors and other community agencies. Demonstrated commitment to continued education through participation in seminars, patient review meetings, webinars, etc. Participation in departmental meetings, inservices, and attention to hospital center memoranda. Strong analytical skills to assess patients needs, develop associated treatment plans, and provide sound advice and guidance.
