Job Description Licensed Practical Nurse (LPN) Location: Bronx, NY Pay Rate: $35.00-$40.00/hr Overview: Seeking a Licensed Practical Nurse to support the health and well-being of youth in residential and foster care. The LPN will coordinate care, administer medication, provide education to youth and families, and ensure compliance with medical standards. Key Responsibilities: - Assess health needs of youth through home visits and direct contact - Coordinate care with physicians and service providers - Administer medications and monitor responses - Educate youth, families, and caretakers on health conditions and self-care - Maintain accurate medical records and ensure regulatory compliance - Participate in team meetings and care planning - Foster a trauma-informed, supportive environment Qualifications: - Graduate of accredited LPN program - NYS LPN license & current CPR certification - Strong communication and teamwork skills - Experience with trauma-informed care preferred - Must be able to travel with youth to appointments
