Job Description:
A company is looking for an LPN Care Coach.
Key Responsibilities
Educate patients on self-management skills and goal setting for
chronic conditions
Implement and improve the Plan of Care, including medication
updates and care coaching
Conduct Transitional Care Management activities to reduce hospital
readmissions
Required Qualifications
Current, unrestricted Compact LPN license
Proficiency with electronic health records and web-based
applications
5+ years of experience as a Licensed Practical Nurse
Ability to work independently and meet performance metrics
Availability to make calls on weekdays during specified hours
