• Manages the delivery of high-quality home care services to patients consistent with agency standards, policies, and state and federal regulations by providing leadership and guidance to nursing staff
• Ensures adequate staffing and appropriate patient assignments
• Maintains the integration of interdisciplinary services
• Facilitates the achievement of agency goals related to clinical outcomes, patient experience of care, fiscal responsibility, and staff retention
  • Current NYS RN (Registered Nurse) license and registration required
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of 3 years' experience in a Certified Home Health Agency; supervisory experience preferred
  • OASIS E; clinical outcome measures
  • Federal Conditions of Participation and NYS DOH regulations related to Home Health Care
